Paperwork
Here at The Dive Bus, we want ALL our divers to be safe AND have the best possible time with us.
As a PADI dive operator, we are obliged to follow the very strict safety standards in order to keep scuba diving a safe sport; failure to complete some of these PADI forms before you leave home could lead to you not able to complete your diving class and incurring charges, so PLEASE take a minute to read this information carefully!!!
- If you're planning to take PADI Discover Scuba Diving class / Intro Dive with The Dive Bus, you'll need to satisfactorily complete the PADI medical statement from you. Since this may require a visit to your physician, you'll should do this before you leave home. Not doing so may result in you not being unable to take your course and still being charged for it...
- If you're planning to take ANY PADI certification dive course with The Dive Bus (other than the PADI Discover Scuba Diving class / Intro Dive) PADI requires us to have a completed Medical Statement from you. Since this may require a visit to your physician, you'll need to complete this Medical Statement before you leave home. Not doing so may delay the start of your course and result in additional charges or even, worst case, mean that you're unable to take your course.
- If you're a certified diver, you will need to complete the Dive Bus waiver. You are welcome to complete this ahead of time although this is not compulsory.
Below, please find all the necessary paperwork for you to download and complete ahead of time. Mandatory paperwork is first; the rest is optional simply to save you time. If you have any problems or questions, please don't hesitate to contact us.

How to download the paperwork:
1. Click on the name of the form to open it (PDF file)
2. 'Save a copy' to your computer
3. Print it out
4. Complete the mandatory paperwork back home. Not doing so could mean you're not able to participate in your activity once you're here :o( Complete the other paperwork back home if you'd like to save time when you check in with The Dive Bus, otherwise complete it when you arrive.
5. Remember to bring it with you!!
